- An aggrieved party may request review by a hearing officer of an order, decision, or administrative penalty assessed by the Investigations and Enforcement Bureau (“IEB”) relative to the interpretation or application of a statute, regulation, or other applicable authority
- An aggrieved party may request review by a hearing officer of any discipline or penalty imposed by a racing official / Massachusetts State Police
Request a Hearing
The Massachusetts Gaming Commission shall afford all parties an opportunity for full and fair hearing. The form below is to be used for requesting hearings by anyone in the Commonwealth’s gaming and racing industries that has been notified in writing of their right to a hearing.
If you have received notice that you have the right to request a hearing and would like to do so, please fill out this form to completion and submit it to the Clerk of the Commission.

Request Commission Review of Hearing Officer’s Decision
Any decision issued by a hearing officer in accordance with 205 CMR 101.02 may be appealed to the Commission for review. An appeal of the decision shall be filed with the Clerk of the Commission on the following form:
